Thank you for taking the time to review the PanCare Transition of Care Plan. Your feedback is valuable in making this document as useful and relevant as possible for young cancer survivors. For each section, a screenshot of the full page is provided, followed by four questions per section. There are no right or wrong answers — we are interested in your honest perspective as a Youth Cancer Council member.

When giving your feedback, please consider the following:

  • Clarity & readability — Is the language clear and easy to understand?
  • Completeness — Is all relevant information included, or is anything missing?
  • Usability — Would a survivor find this section easy and practical to use?
