PanCare Transition of Care Plan
The PanCare Transition of Care Plan supports survivors of childhood and young adult cancer during the transition from paediatric to adult healthcare.
It is intended to be used together with a healthcare provider as a practical discussion tool to guide and support the transition process. Completed jointly by the survivor and healthcare professional, the plan helps structure conversations, capture key information, and provides a clear overview of the transition journey, helping survivors feel informed and confident as they move into adult care.
