PanCare Survivorship Care Plan – Short Version

The PanCare Survivorship Care Plan – Short Version provides a concise overview of treatment history and personalised health recommendations to support long-term wellbeing.

It is intended to be used together with a healthcare provider as a practical discussion tool. The plan supports structured conversations and shared review of medical information, helping ensure that survivors and healthcare professional can jointly address key topics in survivorship care.

The Short Version is a concise version of the PanCare Survivorship Care Plan – Long Version and covers the same core topics. A more detailed Long Version is available for those who would like additional explanation and guidance. The document is available in multiple languages and may be updated over time in line with revisions to the PanCareFollowUp Survivorship Care Plan – Long Version.
