Do you want to set up survivorship care in your clinic or improve what you already have?
Within several EU-funded projects, PanCare has developed helpful tools and resources for the implementation of survivorship care. No matter your starting point, you’ll find practical support here to build or strengthen survivorship care in your setting.
The information on this page can be consulted according to your current needs for implementing long-term follow-up care. If you are at the start of the process we advise to start with the materials from EU-CAYAS-NET. If you have already some survivorship care in place, the PanCareFollowUp materials are a good way to get more information about possibilities to further improve survivroship care care with the PanCareFollowUp Care – and Lifestyle interventions. If have already established long-term follow-up care and are interested in digital tools the PanCareSurPass implementation toolkit can help you with implementing the digital survivorship passport.
